Who should consider Bariatric Surgery?

Candidates of bariatric surgery should qualify the following conditions:

A candidate should have a BMI (Body Mass Index) in between 35-40. The candidate should also possess an obesity-related condition of diabetes mellitus, severe sleep apnea, cardiovascular disease and hypertension. The calculation of BMI is done by dividing the weight of a person in kilograms by height in meters squared.

More Info About Bariatric Surgery

Obesity surgery is suitable for patients with a Body Mass Index over 40 and who can’t lose weight despite regular diet and exercise, for patients with a Body Mass Index between 35 and 40 suffering from diseases linked to obesity such as hypertension, diabetes or sleep apnea. However, when a patient decides to undergo such an operation and is admitted to the hospital, all examinations are carried out in detail.

Apart from these, a Bariatric surgery candidate should also be;

  • Willing to exercise for at least ninety minutes each week.
  • Overweight for more than 5 years.
  • Is able and willing to make lifestyle and eating habit changes.
  • Have had a short-term success with earlier serious attempts for weight loss.
  • Free from auto-immune disease.

Although surgical methods are effective in analysing obesity, surgery is not enough to get rid of weight and to be healthy. After the operation, a serious follow-up period is experienced and the patient needs to restructure all his / her nutritional habits in a way to protect his health and avoid the things that are harmful to his health. It should be kept in mind that reaching the desired weight after the operation is a process that can take months to complete.

Obesity Surgery Methods

  • Intraoral Balloon Insertion

Due to the area covered by the balloon placed in the stomach, the patients has an early feeling of satiety. At Intraoral Balloon Insertion, patients begin to lose weight by eating less. After the patient has lost enough weight, the balloon inside the stomach is deflated with the help of an endoscope. This process takes approximately 6 months. According to their structure, Patients lose over 10-20% of their weight in the meantime.

  • Gastric Banding Surgery

In the adjustable silicone gastric band method, also known as the Gastric Banding Surgery, a band is placed around the stomach just below the junction of the esophagus and the stomach. Thus, the stomach volume is reduced. The band is gradually inflated with serum injections from the port under the skin about 1 month after the surgery. By this means, the size of the gastric passage after surgery can be changed with a simple intervention. This method aims at making the patient feel satiated by consuming very little food.

  • Tube Gastrectomy (Sleeve Gastrectomy)

Gastric Sleeve surgery, also referred to as Sleeve Gastrectomy. Vertical sleeve gastrectomy or longitudinal “vertical gastrectomy” is the removal of 75% of the stomach vertically and transformation of the stomach into a banana or a tube. The procedure is performed laparoscopically with stapler called stapler. Stomach takes the shape of the tube as the continuation of the esophagus. As the volume of the stomach decreases to 25%, it lessens patient’s desire to eat. Patients feel satiated with 1/4 of the quantity of food they used to eat. With this method as bypass is not done in the bowel, absorption deficiency does not occur and less eating causes weight loss. Sleeve gastrectomy came out as a restrictive method of eating. No foreign objects are required, therefore a patient recovers in a relatively short hospital stay.

  • Gastric Bypass

In Gastric Bypass surgeries, a large portion of the stomach is bypassed and a small volume (about 30-50 cc) of stomach is prepared and sutured to the small intestine. In this operation, as in other operations of obesity surgery, the goal is to reduce the volume of the stomach, eliminate a part of the intestines and eliminate some of the food consumed without absorption.Thus, patients feel satiated with less food and a part of the food is absorbed.

SILS (Single Incision Laparpscopic Surgery)

  • SILS is the next generation of laparoscopy, where the access is only through a single port instead of multiple ports. This utilizes new specialized port and roticulator instruments thus making the procedure literally scarless as it will be buried inside the belly button. This procedure though consumes more time, gives early post-operative recovery with less pain than in conventional laparoscopy thus making it a treatment of choice for patients.

Endoscopic Bariatric Surgery

  • There are numerous endoscopic treatment for obese patients. Endoluminal bariatric weight loss surgery is gaining popularity as well, with up to 40% reduction in excess weight loss and around 60 % resolution of co-morbidities. Few of the available procedures include intragastric balloon, primary gastroplasty, and outlet reduction for failed gastric bypass, pouch creation, and endo-barrier bypass sleeve.

Restrictive Procedure – The Lap Gastric Band

The surgery is primarily performed for decreasing the amount of food consumption at a time. The purpose is to create restriction so that a person feels satisfied with little amounts of food. Due to smaller outlet, the food remains in the stomach for a long time. The idea is to decrease daily calorie intake without a feeling of deprivation.

The benefits of gastric band include

  • The stomach returns to its original form even if the band is removed.
  • The recovery period is also slightly shorter.
  • No cutting, removal or stapling of the stomach tissue is done.

The surgeon can easily control the size of the pouch outlet as the band is adjustable.

Malabsorptive procedures-Roux -En- Y Gastric Bypass

This procedure helps in altering the digestion. The length of the intestine is reduced by using Malabsorptive techniques. When the food comes in contact with the intestine, fewer calories are absorbed. Roux-en-Y is regarded as the gold standard procedure for weight loss surgery.

The benefits of Roux-En-Y include

  • Medical conditions like High cholesterol, high blood pressure, sleep apnea and acid reflux are some of the medical conditions associated with obesity. These are either improved or removed after the surgery.
  • About 70-80% of the excess body weight is reduced.
  • The surgery increases the insulin sensitivity and alters the metabolism of carbohydrates. The surgery sometimes eliminates the need for diabetes medications.

The average weight loss is much greater than the average achieved through gastric band surgery and sleeve gastrectomy.
